I don't know what factors are taken into account but I know it's done manually by lichess mods/admins.
To be more precise, mods manually mark streamers as eligible. If such a streamer streams they are then automatically featured on the main page. There is also a tier system, so certain streamers will always appear (i.e. probably people like Magnus Carlsen or maybe official broadcasts) and lower-tiered streamers will be pushed down and may not appear if many featurable streamers are live right now.
It also filters by language, so i.e. Spanish streams only appear for people that have Spanish selected as their Lichess or one of their browser languages. But English streams appear for everybody.
